Do I qualify?

Medicare defines eligibility requirements for home health benefits. If leaving home is difficult, and you are dealing with any of the above issues, we can talk with your doctor and assess your eligibility.

How will I pay?

Home health care is a Medicare Part A benefit that does not require a copay or deductible for patients who meet eligibility criteria. Other payment sources may include Medicaid, private insurance, workers’ compensation, private payment or alternative funding.

It is a program of care and support for individuals recovering from illness, injury, new diagnoses, recent surgery or hospitalization. The focus is on recovery, rehabilitation and health education. Our support team provides a wide range of clinical health services to take you or a loved one home safely and enhance ongoing safety and quality of life.

Medicare Part A (hospital insurance) covers eligible home health services including:

  • Part-time or intermittent skilled nursing
  • Physical, occupational, or speech therapy
  • Medical-related social services
  • Part-time or intermittent hands-on health aide services

You or a loved one may be able to qualify for service with one or more of the following conditions:

  • Recent hospitalization or surgery
  • Safety at home after falls or health changes
  • Multiple medications
  • New diagnoses such as congestive heart failure, COPD, stroke, or diabetes
  • Orthopedic rehabilitation
  • Macular degeneration, glaucoma, or other visual impairment
  • Incontinence
  • Neurological conditions such as Parkinson’s, multiple sclerosis, or cerebral palsy
  • Swallowing issues

Home health care is a Medicare Part A benefit that does not require a copay or deductible for patients who meet eligibility criteria. Other payment sources may include Medicaid, private insurance, workers’ compensation, private payment or alternative funding.

Patients must be under the care of a doctor, or other allowed practitioner such as a Nurse Practitioner or Physician’s Assistant, and getting services under a plan of care that is reviewed regularly by a doctor. A doctor must certify that the patient needs:

  • Intermittent skilled nursing care (other than drawing blood). And/or:
  • Physical, speech or occupational therapy services
