Helen is an 80-year-old grandmother who loves sewing and spending time with her family. About a year ago, she was diagnosed with congestive heart failure. Recently, her symptoms have gotten worse.
Over the past month, Helen has had more trouble walking and moving around. Her feet are swollen, which makes it hard to use her sewing machine and get out to see friends. She also feels weaker and has a hard time standing up from her chair without pulling herself up on nearby furniture.
Helen’s daughter noticed these changes and took her to the doctor. Helen already had medication to help with swelling, but she wasn’t sure when or how to take it the right way. She tried to eat less salt, but wasn’t sure what else to change in her diet.
Her doctor recommended in-home care to help her feel better and stay safe at home.
A nurse came to Helen’s home to check on her and teach her about heart failure, her medications and how to manage her symptoms. She also learned about eating less salt, managing fluids and watching for warning signs. A physical therapist worked with Helen to build strength and improve her balance so she can move around more safely. An occupational therapist helped her with everyday tasks, like using her hands for sewing, saving her energy and moving safely while using oxygen.
With this extra support at home, Helen feels more confident taking care of her health. She is getting stronger, staying safer and looking forward to finishing a special quilt for her new grandson. In-home care is helping her stay independent and giving her and her family peace of mind.
Heart disease is one of the leading chronic conditions affecting adults today. With the right support, many patients can safely manage symptoms, avoid unnecessary hospital visits and maintain independence at home.
