Breathing With Care, our pulmonary disease skilled home health program, is designed to equip COPD patients to successfully manage unique pulmonary diagnosis needs to ease symptoms, lower the risk of complications and improve quality of life.

Interactive Patient Guide

A combined pulmonary patient guide and journal designed to promote conversation, interaction and learning opportunities between patient and clinician. Content is focused on self-assessment, pulmonary disease management and wellness.

Components of the Program

Specific zones are customized for a pulmonary diagnosis so patients and caregivers know when and how to take action.

We are just a phone call away 24 hours per day, 7 days per week. If you have a question or concern we want you to call us for help. We are ready to listen, answer questions, give direction and take action whenever you need us.

Meet Charles

Charles has been living with chronic obstructive pulmonary disease (COPD) for the past two years. He and his wife have been managing his care at home. He uses oxygen when needed and sometimes uses a walker to get around. His wife helps with daily tasks like grocery shopping and cooking.

Recently, Charles hasn’t been eating much and has been feeling more short of breath. He also gets dizzy at times. Even simple activities, like getting in and out of the car, leave him feeling very winded.

One day, while his wife was in the kitchen, she heard a noise and found Charles on the floor. He said he was hurrying to the bathroom when he suddenly felt short of breath and dizzy, then slid to the ground. For a moment, he wasn’t fully awake, but he quickly came around. After resting, he was able to stand and walk again, so they hoped he was okay.

His wife called the doctor, who suggested a breathing treatment and scheduled a follow-up visit. The doctor recommended in-home care to help manage his symptoms and keep him safe at home.

A nurse visited Charles at home to check on him and teach him more about COPD and how to manage his breathing. He learned simple ways to handle shortness of breath and when to take action. A physical therapist began working with him to build strength, improve balance and help prevent falls. An occupational therapist helped make their home safer and showed Charles how to save his energy during daily tasks.

They also connected with support services to help with personal care and light housekeeping, giving his wife extra support.

With in-home care, Charles and his family feel more confident and supported. They now have a plan to help him stay safe, manage his symptoms and continue living at home more comfortably.
