Matters of the Heart: Support for Heart Failure at Home

Living with heart failure can feel overwhelming, but with the right care and guidance, patients can manage their condition, improve their quality of life and remain safely at home.

  • Patient Interactive Guide: A combined heart failure guide and journal encourages patients to track symptoms, practice self-assessment and engage in meaningful conversations with their care team.
  • Disease-Specific Action Plan: Customized “zone” plans outline what to do when symptoms change, so patients and caregivers know exactly when to act and who to call.
  • Call Us First: Around-the-clock access to Elara Caring means patients and families always have a trusted resource 24/7/365 for questions or concerns—helping to avoid unnecessary emergency room visits.
  • ElaraConnect: ElaraConnect’s suite of programs—Engage, Monitor, Check In and Transition— tracks vitals and connects patients to their clinicians virtually.

Skilled Home Health Care: An Added Layer of Support

Because Matters of the Heart is integrated into Elara Caring’s skilled home health services, patients also receive direct, hands-on care from licensed nurses, therapists and other professionals. This includes medication management, symptom monitoring, therapy support and education on heart-healthy living.
