Meet Ada and her family

When 82-year-old Ada enrolled in hospice, her daughter, Lisa, felt overwhelmed.

There were medications scattered across the kitchen counter. Family members had different understandings of what hospice meant. Ada was struggling with pain, shortness of breath and fatigue. Every new symptom created worry. Every question seemed urgent. Everyone was trying to help, but no one felt confident they were doing the right thing.

Ada’s family wasn’t looking for more information.

They were looking for reassurance.

In those first days, the Together, Day by Day approach surrounded Ada and her family with intentional support.

Through early touchpoints, the team took the time to ask questions that mattered most:

“What does quality of life look like for you?”

Ada didn’t talk about medical goals. She talked about sitting on her back porch with a cup of tea. She talked about planting flowers and how much she loved being in her garden. She talked about Sunday afternoons with family gathered around her dining room table.

“What are you hoping for?”

“I just want to feel well enough to enjoy my family,” she said.

“What are you most concerned about?”

Ada worried about being in pain. Lisa worried about making the wrong decisions. The family worried they wouldn’t know what to expect.

“What matters most?”

Being comfortable. Being at home. Being together.

And the team listened.

More importantly, they heard.

Over the next several days, nurses assessed Ada’s symptoms and adjusted medications to improve her comfort. Social workers helped the family navigate difficult conversations and connect with resources. Chaplain support addressed emotional and spiritual concerns. Follow-up calls and visits ensured questions were answered before worries became crises.

  • Instead of uncertainty, the family gained clarity.
  • Instead of fear, they found reassurance.
  • Instead of overwhelm, they experienced calm.

As Ada’s symptoms became better managed, something remarkable happened.

Life began to look more like Ada wanted it to.

She spent mornings on her porch watching the birds. She worked a bit in her garden and even planted some flowers with her grandson. She shared favorite stories around the dinner table and had the energy to enjoy the moments that mattered most.

  • The family no longer spent their days wondering what might happen next.
  • They knew who to call.
  • They knew what to expect.
  • They trusted the team walking alongside them.

Together, Day by Day did more than provide care.

It created connection.

It transformed a rocky beginning into a foundation of trust. It helped Ada and her family move from chaos to calm, from uncertainty to confidence and from simply managing the days to truly living them.

Because the beginning shapes the journey. And when patients and families feel heard, supported and understood from the start, they can spend less time worrying and more time focusing on what matters most.

We are just a phone call away 24 hours per day, 7 days per week. If you have a question or concern, we want you to call us for help. We are ready to listen, answer questions, provide direction and take action whenever you need us.
