ADvantage

The ADvantage home-and community-based services Medicaid waiver program provides services to help people stay at home and in the community instead of going to a nursing home. The program assists older adults and adults who have physical and other disabilities stay independent and close to family and friends. ADvantage promotes individual choice and self-direction to the greatest extent possible.

The ADvantage program does not provide 24-hour care and you must be able to remain safely in your home. For many, ADvantage is a good alternative to other long-term care options like nursing homes.

self-direction to the greatest extent possible.

  • Be qualified financially for Soonercare Medicaid. Maximum income is $2,200 per month for 1 person
  • and cannot have more than $2,000 in checking/savings or assets
  • Meet medical Level of Care (LOC) criteria
  • Be at least age 65 (includes individuals with intellectual/developmental disabilities who are age 65 or
    older) or an adult with physical or other disabilities age 21 to 64 without cognitive impairment
  • Be a resident of Oklahoma
  • Call the ADvantage Careline at 800.435.4711, visit the Oklahoma Human Services website at www.okdhs.org (click on Advantage).
  • You may also apply by printing and completing the Request for Benefits form and mailing or taking it to your local DHS office. If you do not know where your local DHS office is located, you can refer back to www.okdhs.org and click on County Office.
  • Social Security cards or numbers
  • Medicare or insurance cards
  • Proof of US citizenship (birth certificate, voter registration card or US Passport)
  • Proof of immigration status if not a US citizen (legal permanent resident card I-94 or other immigrant registration card, passport or T visa)
  • Life insurance policies; statements of stocks, bonds or certificates of deposit (CDs)
  • Copy of vehicle title
  • Copy of the title and mortgage statement for property you own other than where you live
  • Current benefit check or award letter
  • Copies of child support or alimony check
  • Dated check stubs for the last 30 days that show your name or Social Security number, date of pay, and income before deductions
  • Copy of last year’s tax return only if self-employed, receiving dividends, rental income or mineral rights
  • Bank statements showing savings and checking account balances (be prepared to provide financial information requested for the last five years before application)

If you do not qualify for Medicaid, please contact Elara Caring at 833.463.5272 to discuss other possible funding sources, such as VA benefits, Veteran assistance, private pay, private insurance, PACE or Medicare with United Health Care.
