What is the GUIDE Program?

The Guiding an Improved Dementia Experience (GUIDE) Model is a Medicare program that helps people living with dementia and the family members and caregivers who support them.

The program focuses on improving quality of life, coordinating care, and helping individuals remain safely at home for as long as possible.

How GUIDE Can Help

If you qualify, the GUIDE program may provide:

  • A dedicated dementia care navigator to guide you through your care
  • Care coordination between your doctors, specialists, and community resources
  • 24/7 access to a support line for questions or urgent concerns
  • Education, coaching, and support for family caregivers
  • Respite care to give caregivers a temporary break (up to $2,625 to spend on respite care)
  • Connections to community resources such as transportation, meal programs, and support services
  • A personalized care plan based on your needs and goals

You may qualify if you:

  • Have a diagnosis of Alzheimer’s disease or another form of dementia
  • Have Original Medicare (Parts A & B)
  • Live at home or in an eligible residential setting
  • Are not enrolled in Medicare Advantage, PACE, hospice, or another GUIDE program

  • Support individuals living with dementia
  • Reduce stress for family caregivers
  • Improve communication among healthcare providers
  • Help people remain safely and independently at home

What Is Dementia?

Dementia is a general term for conditions that impact memory, reasoning, behavior and the ability to perform everyday activities.

Alzheimer’s disease is the most common type of dementia. Other forms include vascular dementia and Lewy body dementia. These conditions typically develop gradually and worsen over time.

Many types of dementia lead to symptoms such as memory loss, confusion, wandering, personality changes, difficulty communicating and challenges with daily tasks like dressing, cooking or managing medications. In advanced stages, individuals may require full assistance with personal care and mobility.
