Which In-Home Care Service is Right for Me?

In-home care is a great option for patients who are homebound, managing a chronic illness or are high-risk for COVID-19. Care is provided in the patient’s home setting, and it’s personalized to meet the patient’s specific needs.

Elara Caring provides personal care services, behavioral health, skilled home health and hospice. So which in-home service is right for you or your loved one?

Personal Care: This service helps with activities of daily living, such as dressing, grooming, meal preparation, light housekeeping, medication reminders and errands. Personal care services would benefit someone who has a condition that limits their ability to care for themselves and needs additional support to complete those tasks.

Behavioral Health: In home Behavioral health professionals support patients with a wide range of behavioral health conditions, such as depressive/anxiety disorders, schizophrenia and substance use disorders. The care focuses on patient assessment, education for the patient and family, and action plans that promote recovery.

Skilled Home Health: Skilled home health services include nursing care, physical therapy, occupational therapy, speech therapy, social work and home health aide assistance. If you or your loved one recently had surgery or a hospitalization, received a new medical diagnosis such as CHF or COPD, or has issues with falls, incontinence or low vision, skilled home health can assist with rehabilitation, disease management and health education.

Hospice: End-of-life care is appropriate for a patient with a prognosis of six months or less and when a cure is no longer likely. The in home hospice care team focuses on providing comfort care, such as pain management, spiritual care and home health aide services, and support to the patient’s family. Bereavement services are also available to the family for 13 months after the loss of their loved one.
