Elara Caring’s By Your Side hospice program was created with one goal in mind: to provide compassionate, focused support during the most critical days of the hospice journey. By deepening our presence and care during a patient’s final week of life, By Your Side helps ensure that every individual is treated with dignity and comfort, and that families feel supported every step of the way.

A Deeper Level of Care

Patients and their families have a range of emotional, physical, and spiritual needs at the end of life. By Your Side is Elara Caring’s response to those heightened needs, designed to increase in-person visits and provide more support as patients transition.

Through this program, we enhance our commitment to:

• Manage pain and symptoms so patients remain as comfortable as possible.
• Provide personal care that brings dignity to daily needs.
• Support emotional and spiritual wellbeing through the compassionate presence of our interdisciplinary care team.
• Offer guidance and education to help families feel prepared, heard and never alone.

Our hospice team—composed of nurses, aides, social workers, chaplains and volunteers—comes together to surround patients and their loved ones with reassurance, connection and expert care.

Hospice care is as much about the family as it is about the patient. That’s why By Your Side includes tools and resources that gently guide loved ones through the dying process, helping them know what to expect.
