The Role of a Nurse in Home Care
Nurses make a difference in all health care settings, but their role is especially valuable in home care. Home care nurses visit patients wherever they call home and, at Elara Caring, they can work in skilled home health, behavioral health, personal care services or hospice. A home care nurse’s duties vary based on the patient’s plan of care and the service they are receiving.
Home care nursing services include, but are not limited to:
- Administrating patient medications
- Managing a patient’s plan of care (alongside a patient’s physician or Elara Caring’s medical director)
- Consulting with colleagues like social workers or home health aides to ensure the patient’s needs are met
- Communicating regularly with both the patient’s family and their care team
- Coordinating pain and symptom management
- Observing and assessing the patient’s behavior and health status for changes
- Monitoring and evaluating the patient’s response to treatment or ancillary therapies
- Checking vitals and providing wound care
- Assisting with activities of daily living
- Educating the patient’s family or caregivers on their disease process, care strategies and treatments
