Skilled Home Health Team
Skilled home health care offers a comprehensive range of medical and support services delivered by a multidisciplinary team of professionals.
Your skilled home health team may include nurses, physical therapists, occupational therapists, speech therapists, social workers and/or home health aides.
Nurses (RN or LPN/LVN)
Nurses conduct thorough assessments of patients’ health conditions, including vital signs, wound care and medication management. They collaborate with other team members to develop individualized care plans tailored to the patient’s needs and goals. Nurses educate patients and their families on disease management, medication administration, lifestyle modifications and strategies to promote optimal health and recovery. They also monitor patients’ progress, evaluate the effectiveness of treatments and make necessary adjustments to the care plan to ensure positive outcomes.
Physical Therapists (PT) and Physical Therapist Assistants (PTA)
Physical therapists focus on improving patients’ mobility, strength, balance and coordination through tailored exercise programs and therapeutic interventions. They also educate patients and caregivers on proper body mechanics, assistive devices and techniques to maximize functional independence and prevent injuries.
Occupational Therapists (OT) and Certified Occupational Therapy Assistants (COTA)
Occupational therapists help patients regain or enhance their ability to perform essential daily tasks, such as bathing, dressing, cooking and household activities. They recommend and train patients in the use of adaptive equipment and assistive devices to facilitate independence and improve quality of life. Occupational therapists also assess the home environment and suggest modifications or adaptations to support patients’ safety, mobility and independence.
Speech-Language Pathologists (SLP)
Speech-language pathologists evaluate and treat speech, language, cognitive and swallowing disorders that may result from neurological conditions, stroke or injury. They provide individualized therapy sessions to improve speech clarity, language comprehension, cognitive function and swallowing abilities. Speech-language pathologists also educate patients and families on communication strategies, swallowing techniques, dietary modifications and exercises to enhance functional abilities.
Social Workers (LSW, LMSW)
Social workers provide emotional support, counseling and coping strategies to patients and families facing the challenges of illness, disability and caregiving. They connect patients and families with community resources, support groups, financial assistance programs and other services to address their social, emotional and practical needs. Social workers also collaborate with the healthcare team, caregivers and community agencies to ensure seamless coordination of care and continuity of support for patients transitioning between care settings.
Home Health Aides (HHA, CNA)
Home health aides provide assistance and support to individuals who are unable to perform everyday tasks on their own due to illness, disability or advanced age. Their primary role is to help clients maintain their quality of life and independence. Home health aides assist with personal care, vital sign monitoring and medication reminders.

It is a program of care and support for individuals recovering from illness, injury, new diagnoses, recent surgery or hospitalization. The focus is on recovery, rehabilitation and health education. Our support team provides a wide range of clinical health services to take you or a loved one home safely and enhance ongoing safety and quality of life.
Medicare Part A (hospital insurance) covers eligible home health services including:
- Part-time or intermittent skilled nursing
- Physical, occupational, or speech therapy
- Medical-related social services
- Part-time or intermittent hands-on health aide services
You or a loved one may be able to qualify for service with one or more of the following conditions:
- Recent hospitalization or surgery
- Safety at home after falls or health changes
- Multiple medications
- New diagnoses such as congestive heart failure, COPD, stroke, or diabetes
- Orthopedic rehabilitation
- Macular degeneration, glaucoma, or other visual impairment
- Incontinence
- Neurological conditions such as Parkinson’s, multiple sclerosis, or cerebral palsy
- Swallowing issues
Home health care is a Medicare Part A benefit that does not require a copay or deductible for patients who meet eligibility criteria. Other payment sources may include Medicaid, private insurance, workers’ compensation, private payment or alternative funding.
Patients must be under the care of a doctor, or other allowed practitioner such as a Nurse Practitioner or Physician’s Assistant, and getting services under a plan of care that is reviewed regularly by a doctor. A doctor must certify that the patient needs:
- Intermittent skilled nursing care (other than drawing blood). And/or:
- Physical, speech or occupational therapy services
