What is Transition?

Transition identifies patients on skilled home health that may qualify for more care through hospice.

We look for signs that it could be time for the added benefits so that you and your family have MORE support and MORE time to focus on what matters most.

The timing of electing hospice care is always your decision. And we respect that completely.

There are a lot of myths and misunderstandings about hospice, and we’d welcome the opportunity to have a conversation about this care option, even if you don’t choose it now.

We believe that being informed about the care options you qualify for empowers you to make informed decisions whenever the time is right for you.

An Elara Caring Transitional Liaison (TL) will reach out to simply have a conversation about the hospice benefit.

Our TLs are caring, compassionate and knowledgeable team members who can:

  • Outline the added resources you’ll receive with hospice
  • Discuss eligibility and qualifications
  • Talk with you about common myths and misconceptions
  • Answer questions or concerns

Choosing hospice sooner can mean more quality of life for patients and families, and we want to ensure that you have all of the information you need to make the best choice for you at the time that’s right.

Meet Leon

After a hospitalization three months ago, Leon’s physician ordered skilled home health nursing to help Leon and his wife with symptom management so he could safely stay at home while continuing chemotherapy.

Despite treatment, Leon has lost 25 pounds over the past three months and is experiencing extreme nausea and vomiting. He recently learned that the liver cancer has spread to his lungs.

Mandy, a Transitional Liaison (TL) clinician on the ElaraConnect team, was assigned to Leon’s care and reached out to discuss his personal goals.

This became a turning point is his journey, as he paused to explore what mattered most to him: spending time with his wife and enjoying game nights with grandkids.

The local hospice team provided an informative visit for Leon and his wife at home so they could better understand the added benefits that hospice could provide.

They took some time to think about it and discuss with their children. Their daughter had questions, and Mandy was able to connect with her to address her concerns and highlight the benefits of choosing hospice sooner.

How it's going.

Leon and his family agreed that hospice care was the right choice, and Mandy coordinated everything to make the transition seamless.

Now, Leon is able to remain in his own home with his wife, avoid repeated hospitalizations and experience greater symptom relief.

Because Leon chose hospice sooner, he and his wife are able to enjoy more time together and host game nights with their grandkids as often as possible.
