Hospice care at home is available to patients in New Hampshire at end of life. Our supportive team focuses on the patient’s goals of care and quality of life. Based on the patient’s care plan, we can provide nursing care, hospice aide support, social work services, spiritual counseling and volunteer visits. We also offer 13 months of bereavement support to families.
