Personal Care with the New York Medicaid NHTD Waiver Program

Eligibility Criteria for NHTD

To qualify for the NHTD Waiver Program, individuals must meet the following criteria:

1. Be eligible for Medicaid in New York State.
2. Must have a Patient Review Instrument or PRI completed to determine level of care. This is essentially a “picture” of the patient and determines the patient’s long-term needs.
3. Patient must be Custodial Level of Care or Nursing Home Level of Care.
4. Be able to live safely in a community setting with the support of waiver services.
