Personal Care with the New York Medicaid NHTD Waiver Program

Understanding the NHTD Waiver Program
The NHTD Waiver Program is designed to help Medicaid-eligible individuals who would otherwise require nursing home care to receive services in their own homes or community settings. Services Covered with NHTD
Under the NHTD Waiver Program, participants have access to a variety of services tailored to their individual needs. These services may include service coordination, respite care, day program community integration services and counseling.

Eligibility Criteria for NHTD
To qualify for the NHTD Waiver Program, individuals must meet the following criteria:
- Be eligible for Medicaid in New York State.
- Must have a Patient Review Instrument or PRI completed to determine level of care. This is essentially a “picture” of the patient and determines the patient’s long-term needs.
- Patient must be Custodial Level of Care or Nursing Home Level of Care.
- Be able to live safely in a community setting with the support of waiver services.
