What is PCS Engage?

PCS Engage is a program that allows caregivers to proactively alert the team when they notice changes in a personal care client’s health, helping ensure concerns are addressed quickly and care stays coordinated.

We stay alert to changes so you don’t have to worry alone. If you go to the hospital or something doesn’t feel right, we’re set up to hear about it quickly and step in when needed.

At the beginning and end of every visit, your caregiver answers a few simple questions about how you’re doing. This helps us automatically keep track of any changes in your health and quickly notify the care team if extra support may be needed.

Caregiver supervisors receive the alerts from your caregivers.

Our actions will depend on how serious the type of health change.

We might send a team supervisor or nurse out to check on you in person, or advise you visit your doctor.

Our goal is to help you address small health changes before a major decline occurs.

If a serious health concern is discovered, more drastic action may be advised.

Meet Carmen

Carmen has had Elara Caring PCS caregivers for the past 5+ years after being disabled in a car accident which left her with chronic pain and mobility challenges.

Recently Carmen has been “under the weather” and spending more time in bed. Her caregiver, Simone, noticed redness and skin breakdown when helping Carmen shower.

At the end of her visit, Simone completed her client follow-up assessment via her mobile app which alerted the PCS Engage team that there had been a change in Carmen’s condition.

  • She didn’t have an established primary care doctor.
  • Her pain and ability to walk was getting worse, and she was struggling to leave her home, even for medical appointments.
  • Resources were limited due to financial concerns.
  • Her late husband was a veteran, and she was struggling to access VA benefits.

Actions Taken

  • Connected Carmen with a nearby primary care doctor.
  • In-home nursing was arranged to help with wound care and education, physical therapy for mobility and social work for resource support.
  • Our specialized veteran care team helped Carmen navigate VA benefits she was entitled to as the widow of a veteran.

With these steps in motion, Carmen is now on a better path toward receiving the highest level of home‑based resources available, without navigating the system alone.

PCS Engage helps caregivers already caring for clients at home quickly recognize changes in health, alert the care team in real time and help connect clients to the right support when it’s needed. This coordinated approach helps improve outcomes while supporting clients, families, caregivers and clinicians.
