Skilled Home Health: What to Expect

Skilled home health care is tailored to each patient’s unique needs and circumstances. What can patients expect when they receive home health services?

Initial Assessment

A nurse will conduct an initial assessment to evaluate the patient’s health status, medical history and care needs. This assessment helps determine the services and support required.

Development of a Care Plan

Based on the assessment, a care plan will be developed in collaboration with the patient, their family and their healthcare providers. The care plan outlines the specific services, treatments and goals of care, as well as the frequency of visits and the healthcare professionals involved.

Skilled Nursing Care

Patients may receive skilled nursing care from registered nurses or licensed practical nurses. This may include wound care, medication management, monitoring of vital signs and education about managing their health condition.

Home Health Aide Assistance

Patients may receive assistance with activities of daily living, such as bathing, dressing, grooming, toileting and mobility assistance from home health aides.

Physical, Occupational, or Speech Therapy

Patients who require rehabilitation services may receive physical therapy, occupational therapy or speech therapy in the comfort of their own homes. These therapies aim to improve mobility, strength, function and communication skills.

Medical Equipment and Supplies

Home health services may include the provision of medical equipment, such as mobility aids, oxygen therapy equipment or wound care supplies, as well as training on how to use them safely and effectively.

Education and Support

Patients and their families will receive education and support on managing the patient’s health condition, understanding medications and recognizing signs and symptoms of complications.

Coordination of Care

Home health providers work closely with the patient’s primary care physician and other healthcare providers to ensure continuity of care and effective communication. They may also coordinate referrals to other services or specialists as needed.

Regular Monitoring and Follow-Up

Patients’ progress and health status will be monitored regularly by the home health team, and adjustments to the care plan will be made as necessary. Some patients may also utilize telehealth monitoring services.
