This skilled home health program focuses on heart failure self-assessment, disease management and wellness. We provide an added layer of support with our telehealth program that provides equipment in addition to direct patient care and promotes conversation, interaction and learning opportunities between patient and clinician.
Interactive Patient Guide
A combined patient guide and resource booklet designed to promote conversation, interaction and learning opportunities between patient and clinician. Content is focused on personal goal setting, physical movement, nutrition, education, wellness and medication and heart failure disease management.
Specific zones are customized for heart failure patients to help patients and caregivers know when and how to take action.
