What types of health issues can qualify for services?
You or a loved one may be able to qualify for service with one or more of the following conditions:
- Recent hospitalization or surgery
- Safety at home after falls or health changes
- Multiple medications
- New diagnoses such as congestive heart failure, COPD, stroke, or diabetes
- Orthopedic rehabilitation
- Macular degeneration, glaucoma, or other visual impairment
- Incontinence
- Neurological conditions such as Parkinson’s, multiple sclerosis, or cerebral palsy
- Swallowing issues
