Consumer Directed Services (CDS) is a Medicaid-based New York program offered in Dutchess, Orange, Putnam, Rockland and Ulster counties.

CDS provides personal care attendant services to individuals that meet nursing home level of care, enabling you to live independently in your home, being taken care by people you know and trust.

Choose caregivers you trust the most.

Participants in the CDS program hire their own attendants to assist with tasks of daily living.

Is CDS right for you?

Do you have Medicaid and want to stay in your home? A qualified friend or family member may be hired as an attendant to provide personal care services in the comfort of your own home. What does "consumer controlled" mean?

The CDS program is consumer controlled, meaning you as a CDS client are the employer who hires, fires, schedules and manages your attendant.

Are you providing unpaid care to a loved one and would like to get paid?

Attendants can earn up to $14.75 an hour.

To quality for this program, a person must be:

  • Medicaid eligible
  • 18 years of age or older
  • Able to direct own care
  • Physical disabled
  • Capable of living independently with the services of a personal care attendant

Spouses or anyone under the age of 18 are not eligible to become your attendant.
