Our Philosophy of Caring for the Whole Person

Integrated Behavioral Health

Our care is designed to see each patient's needs as a full-color portrait.

Diagnoses and physical assessment alone can never paint the full picture.

Adding layers of “color” such as mental health status, SDOH and risk factors can eliminate hidden needs and prevent gaps in care.

What is Integrated Behavioral Health?

Our philosophy of caring that recognizes and treats each person as a whole, recognizing that the care of physical and mental health are inextricably entwined.

Integrated Behavioral Health Approach

Our caring clinical team, your home as the preferred care setting, innovative tools and 24-hour availability means that we see you in full color and work tirelessly and intentionally to support wellness physically and mentally.

  • Expert behavioral health nurses are available to assess and address patient’s physical and mental health needs.
  • Physical, Occupational and Speech Therapists utilize a holistic and collaborative approach to their care delivery.
  • Our social workers are equipped to help patients and families find community connections and available resources to address SDOH needs.

We believe the best place for your care is where you live. Treatment which occurs wherever patients call home means that safety and SDOH can be assessed firsthand, environmental risk can be addressed in real-time and care can be individualized to accommodate specific needs.

We utilize person-centric programs and tools that result in engagement and positive outcomes.

  • We ask. Every time. How do we know when our patients need mental health support? We ask. Every visit. Every interaction. Every time. Assessment questions are embedded in our EMR to ensure that our philosophy of care is consistent and scalable.
  • Data Drives Our Direction – We are the only home care agency with an integrated mental/behavioral/physical care model. We are committed to achieving industry-leading outcomes in both physical and mental health.
  • Person-Centric Clinical Tools – We offer a wide array of person-centric clinical tools designed to address both the physical and mental health needs of our patients. (CHF, COPD, incontinence, dementia, anxiety/depression, PTSD, SUD, loss and aging and more)

We are just a phone call away 24 hours per day, 7 days per week. If you have a question or concern we want you to call us for help. We are ready to listen, answer questions, give direction and take action whenever you need us.
