Check In starts after discharge from Skilled Home Health to provide added support and follow-up.
Our team stays in touch periodically by phone to make sure you’re safe and doing well at home.

You’ll receive calls at 14, 30, 60, 90, and 120 days after you are discharged, with added calls if needs are identified along the way.
We’ll ask simple questions about your health, safety and daily needs, and help connect you to support if anything has changed.
Steve was initially admitted to home health after almost falling. After two months, he felt much stronger and steady on his feet after working with a physical therapist on strengthening and balance. He was able to meet his goals, started driving again and was discharged from home health.
He was doing well when the Elara Caring Check In team spoke with him last month, but this time, his situation had changed. He explained to the nurse, Eva that Vivian usually “stays on top of things” but her cataracts were getting worse which was making it more difficult for her. He also admitted that he had swelling in his legs and feet, but didn’t want to worry her.

noticed that both Steve & Vivian may be able to benefit from a Home Health assessment. She contacted their doctor who ordered home health evaluations for both of them.

went out the next day for start of care and found that Steve hadn’t been able to weigh or check his blood pressure because the batteries were dead on his equipment. He was experiencing swelling in his ankles and was admitted to home health care for CHF. A medication change was made to help with fluid retention with education on other ways to reduce symptoms, and what to do if the swelling got worse.
Vivian was struggling to manage her diabetes, largely because of her low vision and was also admitted to home health services. In addition to nursing care for diabetes management, Occupational Therapy was also added to help with practical ways to manage daily tasks even with the limitations of low vision.

Steve and Vivian’s daughter took care of the faulty batteries for Dad and scheduled an eye appointment for Mom. She hadn’t been able to stop by in a couple of weeks and hadn’t realized they were struggling so much. Steve admitted he was considering going to the ER and was thankful for the check-in call. He was reminded that Elara Caring is available 24/7 if he has questions or needs help in the future.
Steve and Vivian are both doing better and enjoying spending time outside together in their garden.
